Confidentiality Agreement & Liability Waiver
The purpose of this volunteer confidentiality agreement and liability waiver is to protect the identity and privacy of individuals and families involved in any accompaniment project or program led by the St. Louis Inter-Faith Committee on Latin America (IFCLA), as well as protect the safety of all volunteers, individuals, and families involved with or connected to IFCLA.
I, (volunteer name), respect the confidentiality of all accompaniment program participants ("compas") and their family members and any secondary contacts provided on any application or anytime thereafter. By signing this agreement, I agree to not reveal any names, facts, biographical information, or other details about any individual and/or their family members and/or secondary contacts to anyone besides the staff and volunteers of IFCLA. I understand that I cannot share any information about the individuals and/or their families and/or their secondary contacts with any third party, unless authorized by the individual in writing. I also agree that I will not ask any individual for any personal information other than described in the vaccination accompaniment process, nor will I ask permission to release their names or any other confidential information in any way. I will not retain or store any personal information of the individual or their families or their secondary contacts on any personal electronic device or in any other electronic or print form. I will maintain this confidentiality indefinitely and permanently, unless otherwise authorized by the individual in writing.
I also understand and agree that it is possible that I may be injured or otherwise harmed during volunteer service due to accidents, acts of nature, my negligent or intentional acts, or the negligent or intentional acts of others; that while IFCLA has taken some steps to reduce the chances of injuries or harm to me as a volunteer, that IFCLA has no control over most risks, and, thus, cannot and does not guarantee nor take any responsibility for the safety of myself as a volunteer or my property while I am engaged in volunteer service; and that I must take full responsibility for myself and assume the risk of harm or damage while serving by taking all necessary and reasonable precautions and acting in a manner that will help protect myself and my property.
While participating in events held or sponsored by IFCLA, including all accompaniment programs, “social distancing” must be practiced and face coverings worn at all times to reduce the risks of exposure to COVID-19. Because COVID-19 is extremely contagious and is spread mainly from person-to-person contact, IFCLA has put in place preventative measures to reduce the spread of COVID-19 in all volunteer activities. However, IFCLA cannot guarantee that its participants, volunteers, partners, or others involved will not become infected with COVID-19. I acknowledge and understand the following: volunteering may include possible exposure to and illness from infectious diseases including but not limited to COVID-19; while particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. By signing this form, I knowingly and freely assume all such risks related to illness and infectious diseases, such as COVID-19, even if arising from the negligence or fault of IFCLA or other volunteers. By signing this form, I hereby knowingly assume the risk of injury, harm and loss associated with this volunteerism, including any injury, harm and loss caused by the negligence, fault or conduct of any kind on the part of IFCLA.
Volunteers agree to self-monitor for signs and symptoms of COVID-19 (symptoms typically include fever, cough, and shortness of breath) and, contact IFCLA at ifcla@ifcla.net if they experience symptoms of COVID-19 within 14 days after volunteering with IFCLA.
I agree to waive and release IFCLA from any and all potential claims for injury, illness, damage, or death which as a volunteer I may have against IFCLA that might arise out of my service and to hold IFCLA harmless there from. If any injury or loss to myself or another does occur due to my intentional actions or due to my negligent actions arising outside of the scope of my volunteer activities, I must accept the liability for and repair, or make reparations for, the harm done. I understand that IFCLA is not providing me with insurance coverage for any injuries, conditions, or losses to me arising out of volunteer activities, except that IFCLA does provide liability insurance coverage as it may relate to any volunteer-led service projects. BY ATTENDING AND/OR VOLUNTEERING IN THIS ACCOMPANIMENT PROJECT, YOU ARE DEEMED TO HAVE GIVEN A FULL RELEASE OF LIABILITY TO THE RELEASED PARTIES TO THE FULLEST EXTENT PERMITTED BY LAW.
I understand and agree to this confidentiality policy and liability waiver. I understand that failing to abide by the terms of this agreement will render me ineligible for future volunteer work with the organization.
